JOB SUMMARY
The purpose of this position is to lead and oversee teams in the development, implementation, organization, ongoing monitoring, evaluation and overall coordination of the ISP and/or IEP plans and transition/discharge plan for children assigned to his/her caseload and provide consultant to other ACC individual’s teams.
The IPC directs teams in carrying out the ISP, IEP programs and leads efforts in troubleshooting gaps, concerns or issues in programs and services and has the authority to direct activity of the teams or individual team members conferring with supervisory staff as necessary.
The IPC directs the accreditation and licensing activities for the ACC residences (homes) of the individual.
The IPC works closely with the Chief Operating Officer and other ACC IPCs in carrying out efficacious ISP, IEP programs.
QUALIFICATIONS (minimum requirements)
The incumbent will have experience in case management and/or a general understanding of the function of the IEP/OSP, Title XIX, and accreditation regulations.
The position requires knowledge in the areas or legal, human and civil rights with medical, rehabilitation, educational terminology and programming with the understanding of laws and rights of persons with disabilities.
Education or years' experience required:
Bachelor’s Degree in Behavioral Science, Social Work, Education or other healthcare related degree
2 years experience in developing, implementing, and monitoring individual plans.
QDDP Certification or willing to obtain.
Preferred/Required Skills and Abilities:
Experience or a working knowledge of Title XIX and accreditation processes
2 years of case management preferred.
Case management of individuals with disabilities preferred.
Supervision of others/decision making aptitude (if applicable):
Direct supervision of other employees will not be a responsibility although overseeing and monitoring that individual plans are implemented and carried out accordingly while providing ongoing feedback and support to staff who carry out the day to day operations and the programming of those served will be a function of this position.
This position carries the specific responsibility in the guidance of Anne Carlsen team members who are implementing programs and solving needs and concerns of the person served.
This position may require training new/existing staff who are carrying out and assisting in writing the plans of the persons served as well as training those who carry out the communications processes, both written and verbal.
