*Position Summary:*
Assesses, plans, implements, and evaluates care for person(s) served under the direction of clinical services director.
Provide support to maintain optimal health so people served can reach their personal goals.
Provide leadership and management of staff to ensure provision of services to the people served in a licensed group home setting.
This position is responsible to: People Supported, Clinical Services Director.
*Key Responsibilities:*
· Conduct ongoing assessments as determined by person’s condition and/or the facility policies, procedures, or protocols and reprioritize care accordingly.
· Document person’s assessment findings and responses to nursing intervention and progress toward problem solution.
· Provide nursing care in a non-judgmental, non-discriminatory manner that is sensitive to the person(s) and family’s diversity, preserving their autonomy, dignity and rights.
· Assist in the development and implementation of the person(s) served plan.
· Encourage person(s) served to participate in the development and implementation of their plan of care.
· Administer medications and perform medical procedures and/or assessments per clinical services director.
· Responsible for ordering medications, monthly reviews, and other needed medical documentation.
· Ensure medical directives are being communicated to staff assisting person(s) served.
· Maintain proper documentation of person(s) served health records.
· Perform staff competencies regarding medication administration, documentation, and MAR compliance.
· Monitor and review all staff who pass medication in the licensed groups homes for proper medication passing technique; conduct periodic observations.
· Provide oversight for medical supply storage, ordering, inventory, and distribution.
· Assure nutritional meals are served by following menus provided by dietician.
· Practice active supports with person(s) served.
· Promote community integration.
· Attend behavioral and service plan meetings when appropriate.
· Supervise and evaluate staff on job performance.
· Responsible for on-site training regarding person served programming.
· Develop and maintain a positive rapport with internal and external entities that are involved in each person(s) served plan of care.
· Protect and preserve operations by keeping information confidential.
· Serve on the Health, Safety, and Wellness Committee.
· Maintains professional and technical knowledge by attending educational workshops.
· Other duties as delegated by the clinical services director.
*Skills and Qualifications:*
Education:
Current LPN license in the state of North Dakota.
Experience:
A minimum of two years active nursing experience.
DD experience preferred.
Other:
1) Active certification in Cardiopulmonary Resuscitation.
2) Flexible and attentive to detail
3) Possess excellent time management and scheduling skills.
4) Strong communication skills
5) Basic computer skills and familiar with electronic medical records.
6) Active driver’s license
7) Able to pass a criminal background check and drug screen.
8) Able to work independently and as part of a collaborative care team.
Physical Requirements:
Strength: Sufficient to assist with lifting and transferring a person(s) and perform CPR.
Ability to lift twenty (20) lbs.
Moving, lifting, or transferring of person(s) may involve lifting up to fifty (50) lbs.
, as well as assist with weights of more than one hundred (100) lbs.
Mobility: Sufficient mobility to bend, stoop and bend down to the floor, ability to move around rapidly and in small, confined areas.
