Full Time or Part Time RN Case Manager

FT or PT Registered Nurse Case Manager needed in Fargo, ND Join Our Team & make a difference at Dakota Home Care! Are you a compassionate and experienced Registered Nurse looking to make a meaningful difference in the lives of patients? Dakota Home Care is seeking dedicated individuals to join our team as Full-Time or Part-Time Nurse Case Managers.
Our Nurse Case Manager position requires an RN degree with at least 3 years of clinical experience.
Experience in pediatric care & home health care is a plus, vent and trach experience helpful.
The Nurse Case Manager is responsible for: Admissions and care planning for clients in their homes, overseeing care to ensure the highest level of quality and support, collaborating with physicians and team members for optimal care delivery, utilizing excellent communication skills to ensure cohesive teamwork.
Proficiency in Microsoft office is helpful.

FT Hours are Monday through Thursday 8 am - 4:30pm and Fridays 8 am - 12pm.
PT Hours are Monday through Thursday 8 am - 2 pm and Fridays 8 am - 12 pm.
