Position Summary
Provision of nursing care to hospice patients in coordination with the interdisciplinary group (IDG) that is consistent with the goals, definitions, philosophy, and purpose of Hospice of the Red River Valley.
All nursing care, as approved by patients’ primary physicians or the hospice medical director, including hospice referrals, patient admissions, physical assessments, symptom management and emotional support.
This position is preferred to be located out of the Hillsboro/Mayville area.
Flexibility is essential to this role, with the typical shift working 9 am to 8 pm, three (4) days per week, based on need.
Will work an occassional weekend and one (1) holiday per year.
Key Responsibilities
Conducts hospice referrals and accurately completes admission paperwork/consents.
Conducts hospice initial assessments at the time of admission.
Assesses the patient’s/family’s needs on an ongoing basis while hospice care is being provided through comprehensive assessment updates.
Develops a plan on care in collaboration with the patient/family, facility staff as applicable, and the hospice IDG that identifies needs, expected outcomes, and interventions for each patient/family.
Maintains an individualized, up to date care plan that reflects the care being provided to patients/families.
Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.
Implements and utilizes physician orders accurately and effectively.
Utilizes levels of care appropriately and shows responsibility for adhering to the Levels of Care policy.
Teaches patient/family in areas related to caregiving and in preparation for death as reflected in documentation.
Patient care updates are relayed to patient care teams at the end of each working day.
Completes RN duties for assignments in various teams: triage, referral/admissions, Referral/Admissions Coordinator, Home team, Hospital Team, Nursing Home Team, Green, Blue, and Purple teams.
Accurately uses expense reports for mileage charged to other team accounts.

Qualifications
Minimum of RN degree; BSN preferred.
Minimum of one year of experience as a hospital staff nurse and/or minimum of two years of experience in health care.
Strong interpersonal communication, counseling, and teaching skills
Flexibility in scheduling; team player
Familiarity with principles related to dying and death strongly preferred
Experience as a hospice nurse preferred.
Familiarity with basic computer functions
Certifications, Licenses, Registrations Required
Current state RN licensure in North Dakota and Minnesota (or willing to obtain dual licensure following offer acceptance)
Current drivers license and auto insurance
