TITLE: RN Case Manager REPORTS TO: Director of Nursing Job Summary: The RN/Case Manager is a registered nurse, licensed by the state, and has experience in geriatric nursing, rehabilitation, safety, critical thinking skills and team management.
Understands the important of being a good leader and team player.
Demonstrates understanding and realizing the importance of each staff person in the TCC team.
Qualifications: Must be registered and licensed with the State of Minnesota as a RN/ and must maintain licensure.
The RN Case Manager is also required to have an ability to read, write and speak English and complete and pass a state background study form which checks for certain criminal convictions or serious/recurring maltreatment.
General Responsibilities: Follows the Traverse Care Center nursing service objectives and philosophy established by the Director of Nursing Services.
Follows standards of nursing practice.
Leads by example with the staff he/she works with on his/her wing.
Follows TCC nursing policy and procedures.
Schedules daily rounds to see and assess residents at their bedside and in the dining room.
Ensures that all nursing services provided to the residents under his/her case management receive adequate and timely care, which is pertinent to the resident’s health status/condition.
Will be responsible to meet the resident/family needs on the wing they are assigned to by doing resident assessment, planning, evaluation and supervision of cares.
Assures the accuracy of resident records.
Participates in the evaluation of staff performance on her/his wing.
Recognizes staff strengths and works toward improving their weaknesses.
Assures that a comprehensive plan of care is established and implemented for each resident and that the plan is reviewed at least quarterly and within seven days of the revision of the comprehensive resident assessment.
Follows the RAI process and consistently uses the MDS user’s manual for guidance on interpretive guidelines.
Seeks out help when warnings come up or needing another opinion to correctly complete MDS.
Careful to follow MDS and RCC schedules.
Interacts with physicians to plan care of residents.
Contributes to identify quality of care, resident rights, quality of life, or facility practice problems.
Assures that nursing personnel caring for residents under her/his case management are following the resident’s care plan.
Does teaching of staff on her/his wing of correct procedures.
Aware of good customer service and demonstrates the use of it.
Uses quality management skills and leadership skills to fulfill the mission of TCC.
Participates in department head meetings and communicates all pertinent information regarding resident care to other departments as each resident’s health status/condition changes.
Communicates to Education Coordinator any/all education/re-education of nursing staff persons.
Represents TCC to other organizations as an organization that is committed to deliver a community of services grounded in faith.
Be especially conscious and careful of this when serving on community committees, boards, and in general friendships.
