The City of Moorhead is accepting applications for an exciting new position called Community Resource Navigator in the Police Department.
This advanced human support position performs difficult work collaborating and communicating across systems to explore and create alternate resources and services for community members experiencing mental health crises and other related problems.
This position will provide post-crisis stabilization and support options for those in need, will collect and analyze data of frequent police calls for service, follow up with and work collaboratively with community service providers on possible case response plans, ensure the appropriate services have been offered to the person in need, and related work as required.
This position is not responsible for responding to crisis calls.
HOURS: Monday-Friday days.
Flexibility.
40 hours per week ESSENTIAL FUNCTIONS The essential functions of the Community Resource Navigator include, but are not limited to the following major duties performed.
Duties are listed from most to least important.
• Communicate with department staff and screen reports to identify individuals in need of follow-up.
• Analyze data to identify mental health needs, series, and trends and assist in establishing resource options for community members in need.
• Develop databases and related structures to maintain and provide information and coordinate appropriate flow of information with partner agencies.
• Assist in facilitating immediate service coordination with community providers and connect individuals to new and existing support.
• Connect and establish successful partnerships with agencies offering short- and long-term support options for community members.
• Communicate and coordinate collaborative response and support options with other area first responders.
• Participate and represent the police department with community outreach needs for community members.
• Provide mental health and community services related resource information to department staff.
• Work with police staff on internal and external needs as needed or directed regarding mental health and community services.
• Performs other duties as requested.
• Attendance at off-site courses/trainings/seminars may be required MINIMUM REQUIREMENTS To perform the Community Resource Navigator position successfully, an individual must be able to perform each essential function satisfactorily.
The requirements listed below are representative of the formal education, experience and training required.
REQUIREMENTS: • Bachelor's degree in social work, psychology, sociology, or related field • Three years of full-time supervised experience in social work, mental health, crisis services, human services, or related • Valid Driver's License KNOWLEDGE, SKILLS, and ABILITIES The Community Resource Navigator requires thorough knowledge of the principles and practices of social work; thorough knowledge of statutes, rules and policies pertaining to social worker and the ability to research the same; thorough knowledge of child and human development, family dynamics and systems, including crisis management and cultural competency; thorough knowledge of community resources and funding sources; thorough knowledge of the principles and techniques of interviewing and recording in social work; thorough knowledge of medical diagnosis, medications, and basic medical terminology, as well as medical insurance processes and procedures; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with associates, other professionals, agencies and the general public; ability to perform and organize work independently; ability to establish relationships with clients and obtain information by direct or collateral interviews; ability to analyze problems presented and develop and carry out effective courses of actions; ability to communicate with and understand the problems faced by clients from all cultural backgrounds and economic levels; ability to prepare case records and reports; ability to use standard office equipment and related software; ability to keep accurate and detailed records.
PHYSICAL REQUIREMENTS and WORKING CONDITIONS The Community Resource Navigator requires frequent exertion of up to 10 pounds of force and occasional exertion of up to 25 pounds of force; work regularly requires sitting and speaking or hearing, frequently requires standing, using hands to finger, handle or feel and repetitive motions and requires walking and reaching with hands and arms; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, operating motor vehicles or equipment and observing general surroundings and activities; work occasionally requires exposure to outdoor weather conditions, exposure to fumes or airborne particles,; work is generally in a moderately noisy location (e.
