Discharge Planner Opportunity
Join the expanding team in our new state of the art 132 bed behavioral health facility!
This full-time opportunity offers 40 hours per week with full benefits.
Prairie St.
John's has been providing services for children, adolescents, and adults to address behavioral health and substance use challenges since 1997.
Prairie offers inpatient hospitalization, day treatment, and intensive outpatient services.
Accredited by The Joint Commission, we are the largest, freestanding behavioral health facility in the region.
More than 4,000 patients receive care from our compassionate health care team each year at Prairie St.
John's.

Qualifications:
Discharge Planner Job Requirements:
· Bachelor’s Degree in Social Work
· Must be licensed or eligible for licensure as a Social Worker in the State of North Dakota
· Must be highly organized and capable of multitasking in a high-stress environment
· Must be knowledgeable in how to handle difficult situations in a way that is best for the well-being of the patient
· Experience using Microsoft Office Products (Word, PowerPoint, Excel and Outlook)
Current license to practice nursing in North Dakota (or eligible to receive or renew).
