00 Department Details Provide outreach to referral facilities and EMS agencies in the tri-state area for AirMed and Sanford Level 1 Trauma Center.
Job duties may entail travel to state-wide conferences, landing zone training and targeted outreach with specific hospitals or agencies.
Candidate will coordinate outreach strategies with both AirMed and Trauma leadership.
Job Summary Identifies key populations to serve, develops relationships with key stakeholders in those populations, identifies the unmet need, and ensures a method to connect patients with appropriate resources.
Responsible for supporting community services and programs that support Sanford Health's vision, mission and goals.
Identifies and partners with advocacy groups to facilitate program development and to support patients through their journey.
The Senior Community Initiatives Specialist works with extensive outside community resources and agencies to develop cooperative working relationships.
Networks with multiple Sanford Health providers and services to develop a comprehensive approach to education and screening.
Provides educational information about the importance of ongoing screening and prevention for a variety of screenings as well as public health offerings and promotions.
Establishes individualized intervention programs, with primary and secondary components, for each patient identified.
Navigates each individual through to resolution using a variety of evidence-based interventions.
Increases awareness of the importance of regular screenings and reduce disparities in screening rates.
Assists with mailings and informational newsletters that are sent to patients.
Establishes good rapport and maintains close communication to ensure goals and objectives are met.
Works directly with patients and clinics to overcome any barriers.
May conduct educational home visits.
Conducts educational sessions at local businesses and may partner with mobile mammography unit to perform on-site screenings.
Conducts community outreach presentations and attends a variety of area health fairs and community events.
Continues to develop a robust, multi-faced approach to improving overall public health through eliminating barriers to care and enhancing education and awareness.
Must have experience in managing multiple projects and working with broad diverse groups of people.
Must have strong critical thinking skills and be self-directed.
Must possess a high degree of organizational and communication skills.
Skillful at managing change.
Experience in marketing and public relations.
Must be able to communicate effectively to a variety of clients.
Qualifications Bachelor's degree required.
Health professional preferred.
Three years experience in engaging with public, program development, project management, and public speaking.
Experience with broad diverse groups of people desired.
Valid driver's license required.
Must maintain good driving record.
