Responsibilities This is a work from home position and it is preferred that you are located in either the Omaha, NE or Fargo, ND metro area.
You will primarily serve our Midwest Region locations across ND, NE, and MN with up to 50% travel.
Summary The Nursing Innovation and Care Transformation Department was initiated as a strategy to create the “Practice Environment of the Future.
” This program management position will collaborate closely across the CommonSpirit Health system with staff and leaders in various markets across regions.
In addition, this role will be a key member of our Nursing Innovation and Care Transformation department, providing innovative strategies through facilitation and management of process improvement initiatives, committees, and care model redesign projects.
This role provides guidance and management for care site and market based Care Transformation and program implementation, sustainment, and advancement in collaboration with Care Transformation regional leadership.
This role will apply professional program management principles/practices and operations development methodologies and tools to guide program development through all phases.
Once implementation is complete, an incumbent retains program management responsibility, continually evaluating operational effectiveness and ongoing viability.
The Care Transformation program manager provides structure for clinical quality improvement within the Care Transformation Team, working in close partnership with nursing, quality, medical and clinical leaders.
In their assigned markets they would be responsible for understanding local current state areas of excellence and opportunities, influencing in order to promote understanding of forward-thinking processes, collaborating and gaining acceptance of recommendations, and achieving overall care model objectives.
This individual manages the analysis of clinical outcomes data in order to develop clinical process improvement initiatives.
Additionally, this position would ensure gaps in practice are addressed and outcomes improved.
Work involves significant interprofessional collaboration with other groups responsible for, or in support of, performance improvement initiatives.
Essential Key Job Responsibilities Manages and coordinates a portfolio of programs for Nursing Care Transformation in support of market leadership, ensuring successful implementation, sustainment, and advancement while aligning with business goals and objectives.
Planning, managing, implementing, evaluating, and facilitating large, cross-functional, complex performance improvement projects and Care Transformation program initiatives in collaboration with organizational leadership/management, hospital staff, medical staff.
Assembles and manages ad hoc cross functional and interprofessional teams from all levels of the organization, unifying efforts toward shared goals.
Maintains program management of Care Transformation programs on an ongoing basis to evaluate performance and achievement of objectives and to identify opportunities for improvements and enhancements.
Analyzes clinical practice/workflows and facilitates innovation and integration into new care models.
Applies accepted standards, tools, and methodologies to uphold data-based performance improvement activities/initiatives; analyzes current problems/processes; collects/analyzes relevant data; uncovers root causes; implements tests of change in a continuous PDSA cycle; change management.
Establish and manage market and care site relationships across multiple departments including leadership, clinical, marketing, human resources, information technology, and project management.
Ability to negotiate change with all levels of management and stakeholders, overcome resistance to that change, and get work done through others in order to effect positive change and generate improvements that can be sustained.
Facilitates/leads meetings with key internal/external stakeholders, prepares meeting materials, and drives effort to identify and resolve issues/risks and determine next steps.
Manages and develops standardized clinical and operational processes across each assigned care site.
Provides recommendations for improvement and written documentation for processes and best practices.
Develop, monitor, and communicate project scope, schedule, and metrics.
Provides consistent and comprehensive status reports and project updates in collaboration with project manager.
This position requires a high degree of professionalism and an ability to interact with a variety of internal and external entities/individuals to develop productive relationships, significantly increase the visibility of care transformation efforts and to facilitate achievement of program goals that respond to organizational strategies and regulatory needs.
Qualifications Master’s degree in a clinical or non-clinical healthcare-related discipline or business Minimum seven (7) years of related work experience in performance improvement and/or program management Demonstrated performance improvement, requisite job knowledge, skills and abilities, including LEAN and/or Six Sigma methodologies Previous experience in an inpatient healthcare setting If clinical, current active licensure Six Sigma Green or Black belt or other process improvement program certification required Comprehensive knowledge of industry services and products High degree of knowledge and capability in change management, LEAN and/or Value Stream and/or Kaizen and/or Six Sigma methodologies Facilitation of process improvement initiatives Demonstrated achievement of large scale projects in healthcare to improve performance and outcomes Demonstrated achievement managing programs Demonstrated healthcare or nursing operations experience Demonstrated ability to drive change, i.
