Lead Clinical Nurse Supervise a team of clinical licensed practical nurses and registered nurses completing clinical services.
Perform all duties of a staff nurse by providing support with clinical visits by identifying patient’s eligibility and execution of guided processes to complete clinical visits.
Assess health care needs that require additional follow up by the primary provider and assist patients in scheduling appropriate follow-up.
Support clinical programs and initiatives, including but not limited to Annual Wellness Visits, Advanced Care Planning Education, Chronic Care Management, and Remote Patient Monitoring.
MAJOR RESPONSIBILITIES: · Direct and supervise clinical nurse team · Champion clinical initiatives including Chronic Care Management, Remote Patient Monitoring, Annual Wellness Visit initiatives, and Advanced Care Planning Education · Ensure team is fully trained across all applicable clinical protocols and company initiatives · Organize schedules to meet the needs of the business · Motivate team to meet and exceed clinical and financial goals · Leverage tools, reports and processes to monitor performance and ensure quality assurance · Serve as a patient advocate and ensure appropriate, reliable and consistent patient care is provided by clinical nurse team · Work collaboratively with clinical teams to accomplish clinical services in an efficient manner for patient and team · Assist patients in connecting with community resources · Address open quality gaps with patients and work collaboratively with patient towards closure of those gaps · Consult with the clinical staff and ancillary department staff to eliminate barriers to efficient delivery of care · Participate in regular team meetings, staff meetings and quality improvement projects to improve patient care · Assist with other clinical duties such as prior authorizations, patient messages, benefits investigations, etc.
KNOWLEDGE, SKILLS, AND ABILITIES: · Working knowledge of third-party pay plans · Excellent customer service and communication skills · Proficient in the English language · Maintain patient confidentiality at all times · Attention to detail and ability to follow policies and procedures · Ability to use Microsoft Office products and execute basic computer skills.
· Ability to handle stressful situations and deal successfully with difficult customers · Ability to maintain effective working relationships with employees and supervisors · Understands laws concerning Continuing Education requirements and takes responsibility to accomplish them · Reliable attendance and punctuality in order to fulfill the essential job functions · Availability to work evenings, weekends, and holidays when needed DESIRED MINIMUM QUALIFICATIONS Education, Licensing, and Experience: · RN degree in nursing · Valid state Registered Nurse license · Current CPR certification · Two years of nursing experience in primary care, community nursing, critical care, or home health is preferred · Chronic Care Management, Remote Patient Monitoring, or Annual Wellness Visits experience · Prior management or leadership positions preferred PHYSICAL DEMANDS: The physical demands described here are representable of those that must be met by an employee to successfully perform the essential job functions of this job.
