Job Description
Job Summary: Principal accountabilities include, planning, supervising, directly participating in and documenting all aspects of the nutritional care of Northland PACE participants.
Performs nutritional assessment and provides nutrition education for Northland PACE participants requiring intervention.
Acts as a resource to IDT.
Coordinates serving of meals, monitors/coordinates kitchen facilities to ensure standards are met.
Coordinates with catering services to ensure catered meals meet needs of Northland PACE participants.
Participates in Quality Assurance actives and committees, where appropriate.
Provides education to other health care professionals, as well as groups, as appropriate.
Primary Responsibilities:
Performs initial nutrition screening upon assessment, developing care plans and monitoring participant progress.
Reassessment is done every six (6) months thereafter.
Interacts with IDT members, as appropriate, to accomplish these tasks at day center, nursing home or participant’s homes.
Monitor monthly weights, taking into account medical diagnosis, drug nutrient interactions, changes in chewing, swallowing, mood changes, hydration status, intake, skin breakdown.
Provides nutrition counseling for Northland PACE participants, using appropriate materials.
Discusses diet principles and other information as appropriate about shopping, food preparation, etc.
Participates in educational activities, developing or presenting monthly group classes; developing or selecting/improving educational materials; or presenting in-services to own department or other departments.
Participates in patient care conferences, multidisciplinary team actives, committees, etc.
as appropriate.
Consults with MD/RNP and staff, making appropriate recommendations; treats all people with respect, maintains confidentiality of Northland PACE participants, and strives towards excellence.
Monitor portion control; appropriate serving of food.
Works on projects for the department, small or large, as appropriate; participates in department quality improvement activities.
Understands and follows Northland PACE Program policies and procedures.
Employment Specifications:
Education/Experience: Minimum of Bachelor of Science in nutrition, and Registered Dietitian.
Minimum of 3-5 years’ experience as a registered dietician, including clinical and administrative responsibilities preferably in geriatrics.
Licensure, registry or certification required: Registered Dietitian with the Commission on Dietetic Registration of the American Dietetic Association.
Special Training: Must be able to work effectively with an interdisciplinary team.
Must be able to communicate effectively.
Must possess strong interpersonal and organizational skills.
Ability to adapt to change in a positive/professional manner.
Must possess the ability to effectively handle difficult and unusual interpersonal situations.
Physical Demands: Must be able to see, hear, and verbally communicate.
Must be able to move about freely and possess manual dexterity and visual acuity.
Moderate amount of walking.
Working Conditions: Primarily inside, well-lit, climate controlled environment.
Hazards of institutional kitchens and potential exposure to infectious diseases and environmental hazards.
Occasional home visits.
Occasional visits to other PACE sites.
May be required to use personal vehicle for business purposes.
