Description: The Licensed Practical Nurse works with other members of the client’s care team to assess for, plan and provide integral health services within the client’s home.
Qualifications:
Pass a Background Check
Hold and maintain active license status as a Licensed Practical Nurse in the state of Minnesota
Maintain a valid Driver’s License and automobile insurance
Job Duties:
Support the mission and Code of Ethics of North Star Nursing.
Serve as a role model for their respective profession.
Provide general health guidance and health teaching to clients and their families at their level of understanding.
Provide client care through assessment and service planning.
Observe, assess, report and document client data to maintain an accurate and timely medical record and report any changes in client mental or physical status to their physician to prevent complications.
Participate in scheduled staff meetings, in-service trainings, and client care conferences.
Promote client and worker safety by enforcing current safety and infection control policies and procedures.
Participate in on-going training and obtain the required number of continuing education units for their respective licensures in order to maintain active status.
Understand the physical, emotional and developmental needs of the populations served.
Communicate effectively with all types of clients and professionals.
Adheres to North Star Nursing’s confidentiality policy in all situations.
Maintains a clean, safe and healthy environment for the patient.
Provides respect for the client, his or her privacy and their property.
Utilizes appropriate and safe techniques in performing transfers, personal hygiene and grooming tasks that include bathing, hair care, skin care, oral hygiene, and toileting and elimination.
Assists with normal range of motion and body positioning.
Provides client with adequate nutrition and fluid intake.
Can be relied upon to be at work consistently and on time when scheduled.
Demonstrates cooperation with scheduling requests to meet client and agency needs.
Is self-directed within their scope of service.

Horizontally transfer lift 60 pounds at 20” height on an occasional basis
Horizontally lift boost 100 pounds at the height of 30”
Grip, grasp and/or twist hands and wrists regularly
